RE Forms and Payments

For each child attending religious education, youth, or any sacramental program, we must have received the proper registration forms and fees. These fees allow us to purchase materials and curriculum.

1. Complete one (1) Annual Medical/Media Release form for each child. This may also be completed online.

2. Make check payable to: St. Ann or pay online
3. Turn in all forms and your payment to the Parish office or Faith Formation office.

*Catechesis of the Good Shepherd registration is FULL. 

Fee Schedule for St. Ann

For Catechesis of the Good Shepherd and/or Religious
Education, or EDGE with NO sacraments:

• 1 child $35
  

• 2 children $55
  

• 3 children $75
  

• 4 or more $90
  

First Communion sacrament fee - additional $55

All High School youth: 

• Per teen $55
  

Confirmation - add $30 per Confirmation teen

Note for Our Lady of the Assumption Mission

If your child will receive First Communion this year, please complete
the First Communion form. Students should be entering 2nd grade to
receive First Communion
